Live Long Well Care
We want our Members to stay independent in their own home and to do that, our Supportive Services Team can help with the activities of daily living right in their own home. Running errands, helping with bathing and dressing, medication reminders and companionship are all available on a pay as you need basis through our sister company, Live Long Well Care.
With their experienced staff providing assistance in the homes of our Members, people at Cascades get the opportunity to regain and retain independence and live their lives in their own homes. With this Supportive Services being provided in conjunction with a healthy diet and participation in our Wellness Program, Members are given a wonderful opportunity to keep themselves from moving to long term care.
Live Long Well Care can provide:
Companionship for people recovering from an illness or accident, or who need more help with the activities of daily living, including:
- Medication reminders
- Meal preparation
- Transportation to doctor appointments or other commitments
- Light housekeeping
Nursing Services for people in recovery or in need of long term assistance to maintain a higher quality of life, including:
- Wound care
- Post surgery care
- Accident recovery
- Pain management
- Certified Nursing Assistance
- Bathing and dressing assistance
Rehabilitation Therapy uses a patient-centered care model to customize physical, occupation and/or speech therapies for physical, social and emotional independence.
Live Long Well Care is licensed, bonded and insured. We accept most private insurances and other third party payment services.
Live Long Well Care licensure is pending currently at Cascades Verdae.
